29 CFR 1910.22(c): Covers and/or guardrails were not provided to protect personnel from the hazards of open pits, tanks, vats, ditches, etc: (a) SOLIDS PROCESSING UNIT, PIT 2 - Employees performed work in the Solids Processing Unit building, and employees were not protected from falling into the North side of Pit 2 which contained hazardous waste, thereby exposing employees to serious injury and fall hazards of up to 10 feet, on or about April 29, 2016. No abatement certification or documentation required.

29 CFR 1910.303(b)(7)(iv): There were damaged parts that could adversely affect safe operation or mechanical strength of the equipment, such as parts that were broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating: (a) SOLIDS PROCESSING UNIT - Employees performed work in the Solids Processing Unit building, and the light switch next to the doorway on the North side of Pit 2 had been struck by the track hoe and damaged, thereby exposing employees to electrical hazards, on or about April 29, 2016. (b) SOLIDS PROCESSING UNIT - Employees performed work in the hazardous waste treatment building, and the switch and electrical conduit that controlled the old heaters for the building, located next to the doorway on the North side of Pit 2 had been struck by a fork lift and damaged, thereby exposing employees to electrical hazards, on or about April 29, 2016. No abatement certification or documentation required.
