29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets:  a. On or about April 26, 2016, the employee handing metal pieces up to other employees on the roof was not wearing a hard hat, exposing himself to head injuries.

Employees installing metal panels at elevations of 6 feet or more above the ground are not being protected from fall hazards by a fall protection system:    The employer fails to assure effective protection against falls as required by the standard, as demonstrated on April 26, 2016, when employees installing a metal roof system on a 3:12 pitch roof at 13 Beech Wood Drive, Ormond Beach, FL 32176, were exposed to an 8 feet 5-inch fall hazard above the ground in that they did not have a fall protection system installed.
