29 CFR 1926.651(b)(4): While the excavation was opened, underground installations were not protected, supported or removed as necessary to safeguard employees.  On or about 28 April 2016 and at times prior thereto, employees were performing excavation operations in a trench where underground utility installations where not supported, exposing the employees to the hazard of being caught in an excavation collapse. A) Two employees were working in the Northwest extension of the trench under and near unsupported utility lines encasement. B) Two employees were working in the Southwest extension of the trench under and near unsupported utility lines encasement.

29 CFR 1926.651(j)(1): Adequate protection was not provided to protect employees from loose rock or soil that could pose a hazard by falling or rolling from an excavation face.    On or about 28 April 2016 and at times prior thereto, four employees were performing excavation operations in an unprotected trench with a face consisting of loose rock and soil, exposing the employees to the hazard of being struck by falling rocks and soil.

29 CFR 1926.651(l): Guardrails complying with 1926.502(b) were not provided where walkways were 6 feet (1.8m) or more above lower levels.  On or about 28 April 2016 and at times prior thereto, six employees were accessing work areas using two earthen walkways to cross over the top of excavations without fall protection, exposing the employees to the hazard of falling from the walkways to the trench bottom below. A) The earthen walkway over the Northwest leg of the trench was approximately 48 inches long by 28 inches wide with fall distances of approximately 12 feet to the West and 15 feet to the East. B) The earthen walkway over the Southwest leg of the trench was approximately 74 inches long by 28 inches wide with a fall distance of approximately 10 feet to the East and West.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c).    On or about 28 April 2016 and at times prior thereto, four employees were performing excavation operations in a trench without protection from an adequate protective system, exposing the employees to the hazard of being caught in a cave-in.  A) Two employees were jackhammering and shoveling while unprotected in the Northwest hole and extension of the trench.  B) Two employees were jackhammering and shoveling while unprotected in the Southwest hole and extension of the trench.
