On or about 28 April 2016 and at times prior thereto, four employees were performing excavation operations in a trench without protection from an adequate protective system, exposing the employees to the hazard of being caught in a cave-in. A) Two employees were jackhammering and shoveling while unprotected in the Northwest hole and extension of the trench. B) Two employees were jackhammering and shoveling while unprotected in the Southwest hole and extension of the trench.
