29 CFR 1926.651(i)(3): Sidewalks, pavements, or appurtenant structures had been undermined and a support system or another method was not provided to protect employees from the possible collapse of such structures:  Location:  Jobsite  Employees were exposed to struck by hazards from asphalt pavement while working in an excavation where the street pavement had become undermined and no support was installed to prevent its collapse.

29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary:  Location:  Jobsite  Employees were exposed to cave-in and engulfment hazards where spoil piles were present on the edge of an un-protected and un-supported excavation.

The employer had not complied with the provisions of 29 CFR 1926.652(b)(1)(i):  Location:  Jobsite  Employees were exposed to cave-in and engulfment hazards while working in a 6'-5" deep trench with straight cut walls where shoring and shielding had been removed.
