29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person. Worksite: The employer did not ensure that all anchorage points that were used for attachment for personal fall arrest systems were capable of supporting at least 5000 pounds per empoyee.

29 CFR 1926.502(d)(16)(ii): The complete personal fall arrest system did not limit the maximum arresting force on an employee to 1,800 pounds when used with a body harness. Worksite: The employer did not ensure that the personal fall arrest system that was worn by an employee who was exposed to fall hazards would limit the maximum arresting force on the employee not more than 1,800 pounds. On or about 04/21/2016, an employee, who was wearing a personal fall arrest system that did not limit the maximum arresting force to 1,800 pounds sustained serious injuries after free falling more than six feet.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level. Worksite: The employer did not ensure that the personal fall arrest system that was worn by an employee who was exposed to fall hazards was rigged in such manner so that the employee would not have more than six (6) feet free fall to lower level. On or about 04/21/2016, the personal fall arrest system that was worn by an employee was not rigged appropriately, and the employee sustained serious injuries from free falling more than six feet to lower level.
