29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:    a-Employees were engaged in roofing activities in which 2 of 3 employees were wearing personal fall arrest harnesses but were not secured via an anchor point exposed to fall hazards of approximately 23ft to the concrete below. Condition noted on or about 04/21/2016  b-An Employee who accessed the roof was not wearing any type of a personal fall arrest harness exposed to fall hazards of approximately 23 ft to the concrete below. 29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing access.    a- Roof:  Employees were exposed to fall hazards of approximately 23ft to the concrete below as a ladder used to access the roof whose side rails did not extend 3ft above the upper landing surface.
