29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    (a) On or about April 27, 2016, Bonds Construction, LLC. did not ensure that employees working in areas where there was possible danger of head injury from impact, or falling objects, were protected by protective helmets. An excavator was being used to move rocks/soil debris from an excavation where employees were observed exposed to struck-by hazards. Employees were not wearing hard hats. A company dump truck was being loaded with rocks/soil debris immediately next to an excavation. Employees were exposed to falling load hazards.
