29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, and the methods and means necessary for energy isolation: a) On or about 05/02/16 in the Mechanical shop; employer failed to protect employees, involved in changing blades on the Doall horizontal and vertical band saws, when the training they provided allowed the emergency stop button to be used as the control method exposing employees to contact with a moving saw blade. b) On or about 05/02/16 in the Maintenance shop; employer failed to protect employees involved in changing blades on the vertical band saw, when the training they provided allowed the emergency stop button to be used as the control method exposing employees to contact with a moving saw blade. c) On or about 05/02/16 in the Mechanical shop; employer failed to protect employees involved in changing the blade on the table saw when the training they provided allowed the disconnect to be shut without requiring a lock to be attached, exposing employees to contact with a moving blade. d) On or about 05/02/16 in the Optics department; employer failed to protect employees involved in setting up the Cart machine (BXY M-32 polishing machine), when the training they provided allowed the machine to be in a manual ( idle ) condition exposing employees to being struck by the rotating head of the polishing machine. ABATEMENT CERTIFICATION REQUIRED

ABATEMENT CERTIFICATION REQUIRED
