On or about 4/8/16 at the second floor north side addition to the existing Willis-Knighton hospital building located at 2600 Greenwood Road, Shreveport, LA; the employer allowed employees to be exposed to the collapse of the second floor deck under construction due to the formwork was poorly designed, erected, supported, braced and maintained so that the vertical shoring could not support the vertical and lateral loads applied to them; thus exposing employees to fall-from-elevation and struck-by hazards associated with the collapsed deck.
