29 CFR 1910.178(a)(5): When trucks are equipped with front-end attachments other than factory installed attachments, it was not requested that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: A. On or about May 11, 2016, outdoors near Powder Silo #0: Yale LP fork lift, Model GLC030CDNUAE077, S/N N473125 was used with a PIVJIB4 boom attachment. The data plate on the vehicle identified load capacity with attachment SA-33B, S/N EGV-EDV, but there was no indication that SA-33B was the boom attachment in use.

On or about May 11, 2016, outdoors near Powder Silo #0: Yale forklift Model GLC030CDNUAE077, S/N N473125 used a 96-inch PIVJIB4 boom attachment, which reduced the rated capacity of the vehicle to 750 pounds. The attachment was used to lift a 1,500 pound part.
