29 CFR 1910.1200(h)(3)(ii): Employee training did not include the physical and health hazards of the chemicals in the work area: (a) Spray painting area, Delavan Automotive- The employer's hazard communication training did not include the physical and health hazards associated with exposure to isocyanates included in the paints and hardeners used by the spray painter. Hazards not covered include, but are not limited to, the signs and symptoms of exposure to isocyanates.

29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: (a) Spray painting area, Delavan Automotive-The employer had not established or fully implemented a respiratory protection program for an employee required to wear a half mask air purifying respirator while spray painting car-hauler trailers. There was no written program, medical evaluation, fit-testing, or training.
