Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards. a) Madison Farms Apartments - on or about May 4, 2016, employees were using a Lull telescoping forklift to raise and horizontally extend a scaffold platform to a working height of approximately 25 feet above grade, but the scaffold platform was not an approved attachment for this model of telehandler. Among other feasible means of abatement, the employer may follow the guidelines of ANSI/ITSDF B56.6-2011, Paragraph 5.15.1a, which states that the employer must "provide a personnel platform which complies with the design requirements listed in Part III of this Standard".
