29 CFR 1926.20(b)(2): The Controlling Contractor did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): Smith Renovations LLC project site located at 824 Superior Ave. Sheboygan, WI: On and before April 29, 2016, Smith Renovations, as the Controlling Contractor, did not ensure the work site was free from hazards which could cause death or serious injuries to subcontractors working on the site, including but not limited to, falls from scaffolding and ladders, struck by falling objects, and noise.
