OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:  The employer is failing to protect employees from amputation hazards associated with using broken or unserviceable equipment.  The most recent occurrence of this was found on the job site located at 4200 South 121st Street, Omaha, NE 68137 where the employer has employees using the tip of their fingers to push in the broken slot pin on the chase turn table for the Mastercut 145 P Die Cutter; thus exposing the employee to an amputation hazard when the table is rotated.    ONE METHOD OF FEASIBLE AND ACCEPTABLE METHODS OF ABATEMENT TO CORRECT THIS HAZARD INCLUDES BUT IS NOT LIMITED TO:    1.     Develop, train employees on, and implement a written work around procedure to manually assist the slot pin through the use of a tool to prevent employee exposure to the amputation hazard.      2.     Implement more stringent safety review on the authorization to use a machine with a damaged or unserviceable part.    3.     Use a tool to depress the slot pin.
