29 CFR 1910.23(a)(1):  Floor openings were not guarded by standard guardrails or covers:    (a) Facility, Cant Deck to the Cooper Head Saw:  There were two approximately 2 foot square floor openings in the decking that employees could fall through.  The fall hazard was about 9 feet to the concrete floor.

29 CFR 1910.213(a)(9): All belts, pulleys, gears, shafts, chains and sprockets and moving parts were not guarded in accordance with the specific requires of 1910.219:      (a)  Facility, Edger Machine:  On or  about May 3, 2016, the chain and sprockets located at the Edger operation's station were not guarded to prevent the operator from getting his fingers/hand caught in ingoing nip points.     (b)  Grading Deck/Infeed to the Irvington Double End Trimmer Saw:  On or about May 3, 2016, the chain and sprocket on the platform end of this machine needs to be guarded against employee contact.
