29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been  taken out of service until restored to safe operating condition(s)    The employer did not ensure that a powered industrial truck found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition, on or about May 4, 2016.     a) Hyster Forklift H155XL (Serial: F006D02861T) located in the storage yard was found to have a defective horn, parking brake, damaged windshield, and damaged side window. The forklift was in operation at the time of the inspection.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d)

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed:    The employer did not ensure that sprocket wheels and chains, which were seven feet or less above platforms were not enclosed. Employees were exposed to amputation injuries from an unguarded chain and sprocket on the conveyor that moves block made by the Tiger machine, located centrally in the plant, to the ovens, on or about May 4, 2016.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d)

Vertical or inclined belt(s) were not enclosed by guards conforming to the requirements specified at 29 CRR 1910.219(m) and (o). The employer did not guard an in-running nip point on a vertical belt used to deliver broken materials to a dumping bin located north of the Tiger machine in the plant, exposing employees to amputation hazards on or about May 4, 2016. An employee was assisting another employee with a repair task by handing the employee tools as the employee inside the Tiger machine conveyor, centrally located within the plant, was working. The assisting employee did not have a lock affixed to the lockout point, on or about May 4, 2016.   Abatement certification is required for this item.
