29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.     On or about May 4, 2016 during the installation of roofing material at the job site located across the street from 821 6TH St SW, Massillon, Ohio 44646, the employer did not ensure employees working on the roof of a residential home were protected from possible danger of eye injury from impact, or from flying objects while installing roofing material.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     On or about May 4, 2016, during the installation of roofing material at a job site located across the street from 821 6TH St SW, Massillon, Ohio 44646, the employer did not ensure employees working on the roof of a single family home were protected from a fall of approximately 8.2 feet by utilizing a fall protection system.
