29 CFR 1910.147(c)(5)(ii)(C)(1): Lockout devices were not substantial enough to prevent removal without the use of excessive force or unusual techniques, such as with the use of bolt cutters or other metal cutting tools: (a) On or about 04/29/2016 - Further Processing Plant, Line 1, Barrel Breader, a machine operator was injured when he bypassed a lock to clean flour from the machine and came into contact with the rotating auger.
