29 CFR 1910.145(c)(2)(i): Caution signs were not used to warn against potential hazards or to caution against unsafe practices:  On or about April 29, 2016 and in times prior thereto, the Caution signs normally used for dump beds were not displayed, exposing operators to the hazard of a free falling truck bed.  The decals state:  NEVER GET UNDER A RAISED BODY FOR ANY REASON WITHOUT THE SAFETY PROP SUPPORTING THE BODY and BE SURE HOIST CONTROL IS IN THE HOLD POSITION.

29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to subparagraphs A-D as follows: A) A specific statement of the intended use of the procedure; B) Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; C) Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; and D) Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures.  On or about April 29, 2016, the Energy Control Program did not have equipment-specific lock-out procedures for such equipment as, but not limited to Mack truck with dump bed, exposing employees to the hazards of unexpected re-energization or release of energy.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control:  On or about April 29, 2016,  a dump truck became out of level on a dirt road while the bed was raised, impairing the dump bed from being lowered.  Dump truck operators/maintenance workers were not trained in the methods and means of energy isolation and control with respect to the function of the hoist and its controls.
