29 CFR 1926.404(b)(1)(ii): 29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupters for personal protection:   a)    Building 171, Inside Machine Shop: On or about 04 May 2016, an extension was plugged into a wall mounted receptacle to supply power a pedestal light that was not provided with a ground fault interrupter device, thus exposing employees to a possible electrical shock hazard.  b)    Building 171, Inside Machine Shop: On or about 04 May 2016, employees plugged a three way GFCI pigtail into the female end of an extension cord then plugged the male end of the extension cord into a wall mounted receptacle, as a result employees were exposed to a possible electrical shock hazard below the GFCI protective device.  No abatement certification or documentation required.

29 CFR 1926.404(f)(7)(iv)(C)(5): Exposed noncurrent-carrying metal parts of cord- and plug-connected portable hand lamps which may become energized were not grounded:(  a)    Building 171 Inside Machine Shop : On or about 04 May, 2016, employees used a portable hand lamp equipped with two conductor wires and an aluminum lamp guard that was not grounded, as a result employees were exposed to a possible electrical shock hazard.  No abatement certification or documentation required.
