29 CFR 1926.405(b)(2): All pull boxes, junction boxes, and fittings were not provided with covers: a) For the construction site at 520 N Falkenburg Rd., Tampa, FL 33619 on the second floor of building 1, as observed on or about 05/05/2016, an electrical box had its cover removed and hanging off of its attachment point such that live electrical wiring was exposed and protruding from the box.
