29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven 7 feet or less above floors or platforms were not enclosed: (a) On or about 04/27/2016 - Palletizer Out Feed, the sprocket and chain, underneath the edges of the horizontal guard of the palletizer nearest the light beam and reflector, were not totally enclosed exposing palletizer operators to amputation hazards.
