29 CFR 1910.24(f): Rise height and tread width were not uniform throughout any flight of stairs including any foundation structure used as one or more treads of the stairs:    On or about May 03, 2016 and at times prior thereto, employees were working on a rig site, H & P Rig #381. At the V-door stairs, the first step was too high off the ground at approximately twenty one inches which was not uniform to the rest of the stairs at approximately twelve inches of rise. This, along with it being offset, created a trip/fall hazard for employees.

29 CFR 1910.303(b)(8)(iii): Electric equipment provided with ventilating openings was installed so that walls or other obstructions prevent the free circulation of air through the equipment:    On or about May 03, 2016 and at times prior thereto,  employees were working on a rig site, H & P Rig #381. In front of the change house, a transformer was installed butted up to the wall with no ventilation clearance. This condition exposed the employees to a fire hazard.
