29 CFR 1910.305(a)(2)(x): Flexible cords and cables were not protected from accidental damage, as might be caused, for example, by sharp corners, projections, and doorways or other pinch points. On or about May 03, 2016 and at times prior thereto, Cactus Drilling Rig #148. Employees were exposed to electrical shock hazards in the following two instances: a) At the Change House/Parts House; a 110v cord was pinched between metal building and door. b) On the back side of the Grasshopper Skid; two 480V cables were pinched in between the metal door and suitcase used to contain some of the rig's electrical wiring.
