29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a)  No. 2 16th Street Wheeling, WV:  On or about May 4, 2016 a new employee was not provided training on the company's fall protection program prior to being sent to engage in fire control duties on the main roof of the building and on the roof of the penthouse.  The employee was exposed to a fall hazard of thirteen feet from the penthouse roof to the main roof of the building without a means of fall protection.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  (a)  No. 2 16th Street Wheeling, WV:  On or about May 4, 2016 two employees were standing on the roof of the penthouse engaging in fire watch duties without a means of fall protection and they were exposed to a fall hazard of thirteen feet to the main roof of the building.  The Quality Steel Erecting, Inc. company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 1056151, citation number 1, item number 1 and was affirmed as a final order on May 4, 2015, with respect to a workplace located at 295 Mall Road in St. Clairsville, OH.
