29 CFR 1926.1408(a)(2): The employer did not determine if any part of the equipment, load line or load (including rigging and lifting accessories), if operated up to the equipment's maximum working radius in the work zone, could get closer than 20 feet to a power line:    a. On or about April 29, 2016, on the east side of the residential building, the employer failed to de-energize and ground the circuit, establish a 20-feet clearance from the 26KV power lines or follow table "A", for the T340 Terex Crane used to set trusses on a residential structure, exposing employees to electrocution hazards.

29 CFR 1926.1408(d)(1): Part(s) of equipment, load line, or load (including rigging and lifting accessories) were allowed below an energized power line:    a. On or about April 29, 2016, on the east side of the residential building, the employer did not confirm with the utility owner/operator that the 26KV power line was de-energized and grounded prior to operating the crane below the power to lift trusses to a residential structure, the load line of the crane made contact with the power line exposing employees to electrocution hazards.

29 CFR 1926.1413(a)(4)(iii)(A): The employer did not remove from service and continued to use the crane without replacing the wire rope after contact with an energized power line:    a. On or about April 29, 2016, on the east side of the residential building, employees rigging, hooking wood roofing trusses and operating a crane were exposed to struck-by hazard because after the wire rope contacted the power lines the employer continue operating the crane and did not removed it from service.

29 CFR 1926.1408(g)(1)(i): At this job site, the employer had not trained the operator of the T340 Terex Crane, serial #12324, in the procedures to be followed in the event of electrical contact with a power line. Such training must include: 1926.1408 A-F.    a.  On or about April 29, 2016, the employer did not train the operator on the procedures to follow in the event of the crane become in contact with power lines while setting trusses, exposing employees to electrocution hazards.
