29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:  On or about May 3, 2016, at the worksite located at Lot 8 Parkhill in Fayetteville, Arkansas,  (a)  wooden boards, cardboard boxes, shingle scraps and other miscellaneous debris were scattered on the ground surrounding the newly constructed home.  These conditions exposed employees to slips, trips and falls from the same elevation.  (b)  wooden boards with protruding nails were scattered on the ground at the front of the newly constructed home.  This condition exposed employees to puncture hazards.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  On or about May 3, 2016, at the worksite located at Lot 8 Parkhill in Fayetteville, Arkansas, employees using pneumatic nail guns to fasten shingles to the roof did not wear eye protection.  Employees were exposed to eye injuries.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about May 3, 2016, at the worksite located at Lot 8 Parkhill in Fayetteville, Arkansas, the portable extension ladder used to access and descend from the roof did not extend 3 feet above the landing surface.  Employees were exposed to falls from elevations.
