29 CFR 1910.119(e)(3)(v): The process hazard analysis did not address the hazards related to facility siting:     INSTANCES (a) and (b) deleted per ISA    a)  For the Styrene Polymerization and Pentane Impregnation Processes - On or about October 17, 2016, the employer failed to address the hazards of an isobutane overpressure on the styrene polymerization and pentane impregnation processes, which were 1500 feet from the isobutane foam sheet extrusion process, and their ability to withstand an explosion overpressure in the 2015 Process Hazard Analysis.      b)  For the Styrene Polymerization and Pentane Impregnation Processes - On or about October 17, 2016, the employer failed to address the hazards of a pressure vessel overpressure on the isobutane foam sheet extrusion process, which was 1500 feet from the styrene polymerization and pentane impregnation processes, and its ability to withstand an explosion overpressure in the 2015 Process Hazard Analysis.      c)  For Fabrication Building 7 - On or about October 17, 2016, the employer failed to address the hazards of a pressure vessel overpressure on Fabrication Building 7, which was within 200 feet of the styrene polymerization and pentane impregnation processes, and its ability to withstand an explosion overpressure in the 2015 Process Hazard Analysis.      d)  For the Styrene Polymerization and Pentane Impregnation Processes - On or about October 17, 2016, the employer failed to address the hazards of a runaway styrene polymerization reactor overpressure on the North Cup Production Building 1, PCC Building 3, Lid Building 4, Switchgear Building 16, and Injection Molding Building 12, which were within 400 feet of the styrene polymerization and pentane impregnation processes, and their ability to withstand an explosion overpressure in the 2015 Process Hazard Analysis.      Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.119(e)(3)(vii): The process hazard analysis did not address a qualitative evaluation of a range of the possible safety and health effects of failure of controls on employees in the workplace:    a)  For the Styrene Polymerization and Pentane Impregnation Processes - On or about October 17, 2016, the employer did not to include the likely number and type of probable injuries and illnesses to employees that could result from the failure of controls, the consequences of a process deviation, or the chemical exposures in the 2015 Process Hazard Analysis.      Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.119(m)(4): A written incident report was not prepared at the conclusion of the investigation that included at a minimum: the date of the incident; date the investigation began; a description of the incident; the factors that resulted from the investigation; any recommendations resulting from the investigation; and the recommendation's resolutions:  a)  For the Styrene Polymerization and Pentane Impregnation Processes - On or about October 17, 2016, the employer's process incident investigation conclusion reports did not consistently include an analysis of the factors that contributed to the incident or any recommendations resulting from the investigation, and a method for the timely documentation and resolution of those recommendations.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
