Abatement documentation and certification is required for this violation.

29 CFR 1926.62(d)(2)(iv): Until the employer performed an employee exposure assessment as required by 29 CFR 1926.62(d), and documented that the employee(s) performing any of the tasks listed in 29 CFR 1926.62(d)(2)(iv) were not exposed to lead in excess of 2,500 micrograms per cubic meter (50 times the Permissible Exposure Limit), the employer did not treat the employee(s) as if the employee(s) were exposed to lead in excess of 2,500 micrograms per cubic meter, and did not implement employee protective measures as prescribed in 29 CFR 1926.62(d)(2)(v): During installation of lead laminated plywood, which included cutting with power saws, approximately during April, 2016, and where no initial determination was made of employee exposure to lead particulates, interim employee protective measures were not taken as described at 1926.62(d)(2)(v)(A) through (F), including respiratory protection, appropriate personal protective clothing and equipment, hygiene facilities, and biological monitoring (determination of blood lead levels). Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. 29 CFR 1926.62(h)(1): All surfaces were not maintained as free as practicable of accumulations of lead: During installation of lead laminated plywood, which included cutting with power saws, approximately during April, 2016, adequate housekeeping was not provided and conducted as indicated by dry sweeping after cutting wall board inside the facility, not directing exhaust from portable air cleaning machines to the outside (resulting in re-entrainment of dust as reported during interviews), and as shown by wipe sampling results where lead dust was found on surfaces in the hot lab, main lobby, and from the top of a tool box next to the main lobby. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citation(s).

29 CFR 1926.62(l)(1): The employer did not communicate information concerning lead hazards according to the requirements of OSHA's Hazard Communication Standard for the construction industry, 29 CFR 1926.59, including but not limited to the requirements concerning warning signs and labels, safety data sheets (SDS), and employee information and training: During installation of lead laminated plywood, which included cutting with power saws, approximately during April, 2016, information and training according to the requirements of 1910.1200(h), were not provided to exposed employees, especially health hazard information from exposure to lead dust, as well as practical controls that could be used to minimize exposure, such as prohibiting dry sweeping of lead contaminated dust, using local exhaust ventilation on power tools, and containment of work at least until exposure determination indicated the level of control necessary. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citation(s).
