OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to potential crushing and struck-by hazards from falling loads, vehicles, and/or lift parts with the following deficiencies:     a)   Auto Service Area, Bay 20, Lift Adjacent to Dispatch Counter:  On or about May 9, 2016, an employee was exposed to the potential of being struck by and/or crushing hazards of a vehicle falling from heights while using the Rotary automotive lift, model number SL210N8GO, serial number RQN10E0011, which had a defective safety air lock cylinder.    b)  Auto Service Area, Bay 20, Lift Adjacent to Dispatch Counter:  On or about May 9, 2016, an employee was exposed to the potential of being struck by and/or crushing hazards of a vehicle falling from heights while using the Rotary automotive lift, model number SL210N8GO, serial number RQN10E0011, which had four damaged arm pads.
