29 CFR 1910.305(a)(2)(x): Flexible cords were not protected from accidental damage, as might be caused for example, by sharp corners, projections, and doorways or other pinch points: On or about May 03, 2016 and at times prior thereto, employees were working at a rig site, Cactus Rig #148. At the front side of the grasshopper skid, a 480V cable was pinched by the metal door of the suitcase. This created an electrical shock hazard.
