On or about May 4, 2016 and at times prior thereto, at or near 1815 Breckenridge Drive, Little Rock, Arkansas, employees were engaged in roofing construction using pneumatic nail guns and hammers without eye protection. This hazard exposed employees to eye injuries.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about May 4, 2016 and at times prior thereto, at or near 1815 Breckenridge Drive, Little Rock, Arkansas, employees used a metal extension ladder to access the roof of the home to perform roof construction. This ladder did not extend at least three feet above the landing surface, exposing employees to fall hazards.

On or about May 4, 2016 and at times prior thereto, at or near 1815 Breckenridge Drive, Little Rock, Arkansas, an employee carried a five gallon bucket in his hand while he descended a metal extension ladder from a single story roof. This condition exposed the employee to fall hazards.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). On or about May 4, 2016 and at times prior thereto, at or near 1815 Breckenridge Drive, Little Rock, Arkansas, employees were working at heights of greater than 6 feet above the ground without fall protection while performing roof construction. This condition exposed employees to the hazards of falls to the ground below. Ramos Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1103208, citation number 1, item number 1 and was affirmed as a final order on 12/14/2015, with respect to a workplace located at 1520 Rockwater Lane, North Little Rock, Arkansas.
