29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and/or maintained in a sanitary and reliable condition: On or about May 04, 2016 and at times prior thereto, employees were working at a parking lot cutting into the pavement without using proper personal protective equipment, specifically hearing and respiratory protection. Employees were exposed to health hazards due to high noise and dust levels.
