29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling.          a)  On or about May 12, 2016, in Building 155, Layout Department, a relocatable power tap energized a skimmer, a radio, and a fan on a daily basis exposing employees to electrical shock hazards.        b)  On or about May 12, 2016, in Building 155, Layout Department, a relocatable power tap located behind the right side of the Chicago Shear used daily to energize a portable radio and a portable battery charger exposing employees to electrical shock hazards.        c)  On or about May 31, 2016, in Building 115, Element Shipping Department, a 4-plex metal receptacle gang box located on the concrete floor, using a plug adapter was energizing a round metal floor fan and the Lantech Stretch wrap machine on a daily basis, exposing employees to electrical shock hazards.        d)  On or about May 31, 2016, in Building 170, Shipping; a 4-plex metal receptacle gang box was not mounted lying on the metal conveyor used daily to energize a fluorescent light and a Ridgid radial arm saw exposing employees to electrical shock hazards.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure.        a)  On or about May 12, 2016, in Building 155, Layout Department, an extension cord ran across the wall to a 2-plug receptacle energizing an industrial box fan in lieu of permanent wiring exposing employees to electrical shock and fire hazards.      b)  On or about May 12, 2016, in Building 155, Hydro Department, a yellow extension cord ran across a hoist to energize a fluorescent tube light in lieu of permanent wiring exposing employees to electrical shock and fire hazards.      c)  On or about June 1, 2016, in Building 197, Carbon Filter Department, two black extension cords were plugged into each other and ran along the wall to energize a stretch wrap machine in lieu of permanent wiring exposing employees to electrical shock and fire hazards.
