29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) The self-cleaning tail pulley on the conveyor feeding concrete mix to the Texam box culvert mold caster was not guarded to protect persons from contacting rotating parts and an ingoing nip point. The conveyor was elevated approximately 5 foot above the floor level and could move in and out and side to side filling a mold with material. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19
