On or about May 6, 2016 and at times prior thereto, at or near 7609 Vestal Blvd., North Little Rock, Arkansas, an employee was working at heights greater than 6 feet above the ground below performing job tasks such as but not limited to a damage inspection for leaks. This condition exposed the employee to the hazard of falling from heights.
