OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to potential falling hazards due to modified working surfaces.      a) West end - on or about May 9, 2016, employees used a mobile ladder stand to access the second floor mezzanine, but the employer had installed a wood extension placed on the platform to obtain additional height, exposing employees to fall hazards.    Among other methods, one feasible and acceptable method to correct this hazard, is to ensure additional height is not gained by the addition of any type extension or object being placed on the mobile ladder stand in accordance with the American National Standards Institute (ANSI) A14.7, Section 6.7 which states, "Additional height shall not be gained by the addition of any type extension or an object being placed on the unit".
