29 CFR 1910.23(c)(1): Open-sided floor(s) or platform(s) 4 feet (1.22 m) or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides: a. On April 28, 2016, on the wet line marinate tumbler, the platform designed to allow employees access the marinate tumbler did not have an adequate railing, exposing employees to a 5 and 1/2 feet fall hazard.

29 CFR 1910.23(c)(3): Regardless of height, open-sided floors, walkways, platforms, or runways above or adjacent to dangerous equipment, pickling or galvanizing tanks, degreasing units, and similar hazards were not guarded with a standard railing and toe board: a. On April 28, 2016, in the box room, the platform used to load box components did not have an adequate guardrail, exposing employees to the hazard of falling into dangerous equipment.
