29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    (a) Laundry Department:  On April 25, 2016 and times prior, employees change out 5 gallon buckets of Ecolab Tri Star Neutralizer and Laundri Destainer, which are corrosive.  There are no suitable facilities for quick drenching or flushing of the eyes in the immediate work area.      ABATEMENT NOTE:  Suitable facilities for quick drenching or flushing of the eyes consists of 15 minutes of continuous flow of water.  The facility must also be located within 100 feet with an unobstructed path from the corrosive material(s).

29 CFR 1910.1200(e)(1):   The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:     (a) Facility Wide:  On May 6, 2016 and times prior, employees used chemical products during hotel operations including but not limited to, Ecolab Laundri Destainer, Ecolab Heavy Duty Bathroom Cleaner, and Maxim XXXtractor carpet cleaner. The employer has not developed or implemented a written hazard communication program.     ABATEMENT NOTE:  A written program shall include a description of how the criteria for the following will be met:   1.        Labeling and other forms of warnings;   2.        Safety Data Sheets;   3.        Employee information and training.        Additionally, a list of hazardous chemicals known to be present in the workplace must be compiled.  Methods used to inform employees of the hazards associated with non-routine tasks and the informing of contractors of workplace hazards, including a description of the labeling system used in the facility and the availability of safety data sheets must also be addressed.  The written program must be made available upon request.

29 CFR 1910.1200(h)(3)(iv):   The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:     (a)  Facility Wide:  On May 6, 2016 and times prior, employees used chemical products during hotel operations including but not limited to, Ecolab Laundri Destainer, Ecolab Heavy Duty Bathroom Cleaner, and Maxim XXXtractor carpet cleaner.  The employer has not provided employees with information and training on the new label elements (pictograms and signal words) and the new safety data sheet format as required per 29 CFR 1910.1200(j)(1).

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:    (a)  Maintenance Department:  On April 6, 2016 and times prior, employees voluntarily wore a 3M 8511 N95 tight fitting respirator (filtering facepiece) during maintenance activities.  The employer did not provide employees with the information contained in Appendix D of the Respiratory Protection Standard.
