29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): a) In the northeast section of the outside yard area, a 110 volt green extension cord being used to supply power to a portable circular Skilsaw, had its outer insulation pulled away from its orange plug, allowing strain to be placed the inner electrical connectors.

29 CFR 1910.24(c): Fixed stairway(s) were not designed and/or constructed to carry a load of five times the normal live load anticipated or the strength to carry safely a moving load of 1000 pounds: a) On the concrete steps located on the southwest end of the concrete batch plant, with a platform 32 inches high with 8 inch risers, was damaged on the first two steps creating a trip hazard while decreasing its strength capacity.
