29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a)20 Begley St., Agawam, MA: On or about March 21, 2016, an employee was decking the upper floor of the residence being constructed, more than 11 feet above the ground below without any measures to protect him from falling to that lower level.
