29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder: (a) On or about April 20, 2016, employees were exposed to fall hazards in that the portable ladder did not extend 3 feet above the upper landing surface and/or was not secured at its top to a rigid support. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) On or about April 20, 2016, and at times prior, Cornerstone Roofing and Gutter did not ensure that employees were protected from falls of 9.5 feet and greater while engaged in residential roofing operations. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet"). Cornerstone Roofing and Gutter was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1058154, citation number 1, item number 1a and was affirmed as a final order on July 22, 2015, with respect to a workplace located at 2116 Farnon Ct, Colorado Springs, CO 80906.
