29 CFR 1910.119(f)(3): The operating procedures were not reviewed as often as necessary to assure that they reflect current operating practice, including changes that result from changes in process chemicals, technology, and equipment, and changes to facilities: (a) Blending Operations Procedure BL0001 - Tote Tilt Procedure: The tote tilt procedure identified in section 12 contained directions to use the tote tilt stand for draining totes. The current procedure for draining totes requires personnel to use forklifts to drain totes. (b) Blending Operations Procedure BL0001 - Mixer Blade Cleaning: The mixer blade cleaning procedure identified in section 9.17 contained directions to use the yellow cardboard cover to protect the mixing blade while not in use. The current procedure is to lower the mixing blade into an empty steel 55 gallon drum to protect the mixing blade when not in use.

29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy. (a) The written Lockout/Tagout Posted Procedure for the T-14 Primary Transfer Pump stated the need to ensure that pressures were isolated and/or relieved but did not state specifically the method to isolate / relieve the hydraulic pressure within piping systems.
