29 CFR 1910.333(a)(1): Live parts to which an employee was exposed were not deenergized before the employee worked on or near them: a. On or about May 5, 2016, at the above addressed jobsite, an employee was severely injured after contacting live parts of a 1000 Watt metal halide bulb(s) which included, but was not limited to a 480 volt circuit. The electric was not deenergized prior to the employee working near and/or on the live part.

29 CFR 1910.335(a)(1)(i): An employee working in area where there were potential electrical hazards was not using electrical protective equipment that was appropriate for the specific parts of the body to be protected and for the work to be performed: a. On or about May 5, 2016, at the above addressed jobsite, an employee was required to replace a 1000 Watt metal halide lamp bulb(s). The employee attempting to replace the bulb on a live part, including but not limited to a 480 volt circuit, did not use electrical protective equipment including voltage-rated gloves and flame resistant clothing. The employee was thereby exposed to the hazards associated with live electrical parts.
