Building 49150, machine shop; on or about 10 May 2016 and times prior thereto; the employer did not ensure that the Brown-Brockmeyer bench grinder, model 709CS-1, serial number 3704266, was securely mounted to prevent movement. This condition exposed employees to a struck by hazard.

29 CFR 1910.254(d)(9)(iii): Cables with damaged insulation or exposed bare conductors were not replaced.  Building 49150, Welding shop, Room D-110; on or about 10 May 2016 and times prior thereto; a Miller Dimension 452 multipurpose welder, serial number LF280073C, Trak number 8TCAA111 had damage to the outer insulation jacket on the welding cable. Cable had a cut located at approximately 60 inches from the stinger, exposing electrical wiring. This condition exposed employees to an electrical shock and burn hazard.

29 CFR 1910.303(b)(7)(iv): There shall be no damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating.  Building 49150, Room D-109; on or about 10 May 2016 and times prior thereto; a 12 gauge yellow heavy duty indoor/outdoor extension cord had damaged to the outer jacket. This condition exposed employees to an electrical shock and burn hazard.

This condition exposed employees to an electrical shock and burn hazard.
