29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:    On or about May 6, 2016, at the above addressed site, employees were exposed to a fall hazard of approximately 18 feet 9 inches while placing discarded roof tiles into the temporary elevated dump body of a truck while not being protected from falling to the lower level.

29 CFR 1926.503(c): The employer did not retrain affected employees  who already had been trained but demonstrated inadequate  understanding and skill required by paragraph (a) of this section:    On or about May 6, 2016, at the above addressed site, employees exposed to fall hazards while performing roof work who already been provided with training were demonstrating inadequate understanding and skill to properly protect themselves from falling prior to having the employer retrain them.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    On or about May 6, 2016, at the above addressed site, employees were exposed to a fall hazard of approximately 11 feet 6 inches to  13 feet 6 inches when performing roof work removing existing roof tiles from a residence while not being protected from falling.    CARPENTER'S ROOFING & SHEET METAL, INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVILANT STANDARD 29 CFR 1926.501(b)(11) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 934332, CITATION NUMBER 1, ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER ON 02/05/2014, WITH RESPECT TO A WORKPLACE LOCATED AT 1564 SOUTH OCEAN BOULEVARD IN PALM BEACH, FLORIDA.
