29 CFR 1910.178(l)(1)(ii): Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer did not ensure that each operator had successfully completed the training required by this paragraph (l), except as permitted by paragraph (l)(5):    On or about May 11, 2016, an employee operating a rough terrain forklift was exposed to struck-by hazards, because the employer did not ensure the operator had completed training, that included all of the truck and workplace related topics listed in the standard and completed an evaluation of the operators performance through practical exercise.

29 CFR 1910.178(l)(6): The certification did not include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation:    On or about May 11, 2016, an employee operating a rough terrain forklift and the employer did not document the required certification for each operator, which includes:   the operators name, date of the training, date of the evaluation and/or the name of the person conducting the evaluation.

29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s)    On or about May 11, 2016, employees performing roofing activities were exposed to fall hazards on a construction site in Peoria, Illinois, and the employer did not ensure a competent person inspected the job site for unsafe activity and took necessary steps to eliminate or control fall hazards.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.   On or about May 11, 2016, employees performing roofing activities were exposed to fall hazards on a construction site in Peoria, Illinois, and the employer did not adequate fall protection was being used.    a.  Employees were exposed to a fall hazard of approximately 14ft., because the warning lines were not erected around all sides of the roof.   b. Employees were exposed to a fall hazard of approximately 34ft., because employees were working outside the warning line.      Superior Singleply, Inc. previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection number 895371 citation number 1, item number 2, and was affirmed as a final order on May 9, 2013, with respect to a workplace located at 5409 N. Jim Miller Road, Dallas, TX 75228.
