On or about 5/10/2016, GMP Metal Products had not ensured that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training.  A manager, supervisor and two employees were operating powered industrial trucks without completing the required training.

GMP Metal Products had not examined powered industrial trucks daily.  The following forklifts were not examined before being placed in service:     (a)  Toyota Forklift, Model 8FGCU30 - Serial # 15098, had worn pads on the pedals and cardboard attached to the overhead guard, which limited or obstructed the operators view.     (b)  Toyota Forklift, Model 8FGCU25 - Serial # 31366, had the LPG tank installed incorrectly; locator/indexing pin was cut off.  The unit had cardboard attached to the overhead guard, which limited or obstructed the operators view.
