29 CFR 1926.28(a): The employer did not require the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions:  a) 3901 / 3905 West Main Road, Vera Cruz, PA, Worksite - on or about May 10, 2016, employees were receiving roofing materials being lifted to the working height by a knuckle boom loader truck, which creates an overhead strike hazard, but the employees were not wearing any type of head protection.
